Clinical Assessments & Care Planning
Experienced Baycrest@Home social workers are available to assess your family’s needs and help you navigate the health system, manage complex family dynamics, and prioritize issues. For both urgent and longer-term issues, we’ll give you concrete and actionable next steps.

Home Safety Assessments
Baycrest@Home occupational therapists are ready to increase safety, decrease the risk of falls, and improve overall home safety and comfort. Recommendations can be for specific home areas and can range from simple changes to larger modifications using in-home technologies.

COUNSELLING & ADVICE

Timely & Urgent Issues Counselling
As timely and urgent issues arise, Baycrest@Home members have access to social work, occupational therapy and nurse experts to ensure their questions and concerns addressed by experts informed by both professional insight and knowledge of your unique situation.

On-Going Individual & Family Counselling
Our social work team is skilled in providing support around caregiver stress and competency and resilience building, loss, grief and bereavement, as well as guiding you in managing complex family dynamics. Counselling can be one-on-one or with your family members.

Nursing Advice
Connect with a Registered Nurse to ask questions about emerging medical issues and get advice on how to deal with difficult behaviours, reduce medication errors, cope with sensitive issues such as incontinence, or how to improve overall health and wellness.

EDUCATION & CONNECTION

Caregiver Skills Webinars
Baycrest@Home supports skills building with information and training for busy caregivers, in a variety of formats. Our skills series ‘Caregiving 101’ is led by professionals and helps caregivers feel better equipped with skills to care for aging adults with cognitive changes.

Caregiver Support Groups
Facilitated by experts, group sessions bring caregiver types (spouse, daughter/son) together to share advice, experiences and support. These groups give caregivers a chance to connect with others on similar journeys and reduce the sense of isolation that comes with caregiving.

Caregiver Resource Library
We offer a comprehensive self-serve library of pre-recorded videos, reference materials and articles curated to give caregivers resources and news, when you need or want them.
